Please provide us a video proof as this is vendors requirement for a refund. Video Should consist of:
a) Showing the IMEI by dialling *#06#
b) Showing the phone Model Number, Android Version, Kernel Version and Build Number
c) Entering the unlock code and then the message shown on the screen
d) Upload the video to youtube and send us the link or email us the video

Video Proof If Code Is Not Working: (For Samsung Galaxy S3 and Samsung Galaxy Note2 only)

For Samsung Galaxy S3 and Samsung Galaxy Note2 please make a single video for the above steps. And also make a video for the following steps.

a) Go to the Dial pad and dial *#197328640#
b) Please click on number [1] UMTS > [1] Debugging screen > [8] Phone Control > [6] NETWORK LOCK
c) You will see 3 options
d) Please click on number [1] Perso Sha256 INFO, the screen will change slightly
e) Please hit the MENU button then when the menu pops up please carefully hit BACK to return to the previous screen
f) Please click on number [2] Perso Sha256 ON, the screen will change again to a one line message
g) Restart the phone on the screen by holding the power button and click RESTART
h) When the phone prompts for the unlock code simply enter the unlock code (Provided by us)
i) Your phone is now permanently unlocked!

Video Proof If MetroPCS or T-Mobile Phones are not Unlocked

This requirement is for Mobile Unlock Apps only. Make a video for the following steps.

a) IMEI# of the Phone while composing *#06# on your keypad (Must show IMEI# on the screen)
b) Error while using Device Unlock App (must show “Not Eligible for Unlock”)
c) Write Unique reference: (Provided by us) on a piece of paper. And put it besides the phone. This paper with reference# should be shown while recording the video.
d) Record all the above steps and upload it on youtube.
